ABSTRACT

Abstract Local anesthetics are essential medications for the conduction of dermatological procedures. They stop the depolarization of nerve fibers and are divided into two main categories, the amide and ester types. Systemic toxicity with reflex on the central nervous and cardiovascular systems is their most feared adverse reactions, and the anaphylactic reaction is the most concerning one. Although potentially fatal, these events are extremely rare, so local anesthetics are considered safe for use in in-office procedures.

ABSTRACT

A hipercurvatura transversa é deformidade da placa ungueal, inestética e dolorosa, provocando desconforto ao paciente. O tratamento pode ser conservador ou cirúrgico. Dos conservadores a colocação da lâmina plástica de memória sobre a placa ungueal, utilizando cola de cianocrilato, promove rápida melhora da dor e seu alargamento entre três e seis meses. O uso de acelerador de cianoacrilato facilita o procedimento tornando-o mais rápido.


Transverse hypercurvature is a deformity of the nail plate, unaesthetic and painful, causing discomfort to the patient. Treatment can be conservative or surgical. Related to the conservative treatments, placing the flexible plastic blade on the nail plate using cyanoacrylate glue, promotes rapid improvement of pain and its widening from three to six months. The use of cyanoacrylate accelerator facilitates the procedure, making it faster.

ABSTRACT

A sixty-one year old white female was referred to the Dermatology Department to treat an ingrown nail in the inner corner of the left hallux. Examination of the entire nail unit showed the presence of xanthonychia in the outer corner besides thickening and increase in the transverse curvature of the nail plate. Dermoscopy and nuclear magnetic resonance of the free edge of the nail plate detected characteristic signs of onychomatricoma, a diagnosis that was later confirmed by anatomopathological exam.

ABSTRACT

Onicomatricoma é neoplasia benigna específica do complexo ungueal descrita em 1992, sendo o único tumor em que a alteração da placa ungueal é produzida ativamente pela lesão. Encontra-se na matriz ungueal apresentando projeções digitiformes que ficam impressas na placa ungueal, resultando em espessamento, estrias longitudinais, coloração amarelada e hemorragias em estilhaço. Apesar de descrito como tumor raro acredita-se que seja subdiagnosticado. O objetivo deste artigo é revisar as características clínicas e os exames complementares subsidiários para o reconhecimento e diagnóstico desse tumor.


Onychomatricoma is a benign neoplasia that occurs pecifically in the nail apparatus. It was described in 1992, and is the only tumor in which the change of the nail plate is actively caused by the lesion. It lies in the nail matrix,and has fingerlike projections that are embedded in the nail plate, resulting in thickening, longitudinal grooves, yellow discoloration and splinter hemorrhages. Although described as a rare tumor, it is believed to be underdiagnosed. The present study is aimed at reviewing onychomatricoma's clinical features as well as the complementary examinations that are used to recognize and diagnose this tumor.

ABSTRACT

An 80-year-old Caucasian male patient was referred for evaluation of a rapidly growing, asymptomatic, erythematous nodule measuring 2 cm in diameter on his left cheek. The lesion had been present for four months. Dermoscopy revealed a homogeneous pink background with polymorphous telangiectatic vessels. Histopathology showed tumors in the deep dermis and subcutis composed of round cells with scant cytoplasm. Immunohistochemical staining was positive for CK20 confirming the diagnosis of Merkel cell carcinoma.


Paciente de 80 anos, branco, sexo masculino, encaminhado para avaliação de nódulo eritematoso de 2 cm, assintomático, de crescimento rápido, localizado na região malar esquerda, com quatro meses de evolução. À dermatoscopia visualizou-se fundo homogêneo róseo com telangiectasias polimorfas. O exame anatomopatológico revelou massas tumorais atingindo a derme profunda e o subcutâneo, compostas por células arredondadas, de citoplasma escasso. A imunohistoquímica foi positiva para CK20, confirmando o diagnóstico de carcinoma de células de Merkel.

ABSTRACT

Queloide é afecção descrita há alguns séculos, porém ainda hoje seu manejo e terapia apresentam resultados muitas vezes insuficientes. Há, atualmente, diversos tratamentos, como corticoterapia intralesional e terapia compressiva, entre outras. Relata-se o caso de paciente com histórico de queloides nos lóbulos das orelhas há cinco anos, com evolução recidivante após as terapêuticas empregadas. Utilizando injeções intralesionais de bleomicina após redução cirúrgica, houve regressão completa das lesões por dois anos. A bleomicina tem-se tornado opção terapêutica para queloides refratários aos tratamentos convencionais com poucos efeitos colaterais e resposta duradoura.
